Assistant Medical Director

The goal of our service model is to operate within skilled nursing facilities for the benefit of the residents, the nursing facility operators and the facility medical staff. Through intensive training, benchmarking of outcomes, strong direct communication with the facility leadership team, and partnerships with the facility medical staff, Integritas developed the Assistant Medical Director (AMD) Model, an integrated interface between the medical practitioners and the skilled nursing facilities.

The Roles and Responsibilities of the Assistant Medical Director include:

1. Providing Physician Concierge Services:
a. Establishing a working relationship with the Facility Medical Staff to act as “Medical Customer Service” making it           easier for the Attending to provide patient care.
b. Communicate with Attending Physicians regarding care plan changes on his/her patients.
c. Facilitating pharmacy Formulary Compliance
d. Monitoring lab and diagnostic data to ensure appropriate follow-up
2. Monitoring the medical components of the facility Quality Assurance Program which include:
a. Being involved in the assessment and documentation of the facility’s clinical quality indicators including:
• Skin integrity
• Infectious disease incidence
• Psychotropic medication usage
• Pain management
b. Monitoring and analyzing the trends related to the Quality Assurance data to facilitate root cause analysis.
c. Transmitting Quality Assurance data to the facility Medical Director and Quality Assurance committee for program analysis or modification
3. Identifying areas of clinical need and assimilating it into Facility In-service Training and scheduled Disease Management visits.
4. Supplementing Medical-Legal Risk Management by:
a. Documenting according to clinically accepted documentation guidelines
b. Updating the resident family members regarding patient status as needed.
c. Notifying facility Administrator and Director of Nursing of all identified incidents or care concerns. Let us be your medical services partner!!
